This form is for students currently registered with the Glenn Roy Blundon Centre for Students with DisabilitiesOne completed form must be submitted for each requested item, and filling out this form will initiate a request.

Turnaround time is on an item-by-item basis; due to this, students are encouraged to submit a request as soon as the item information is known. Please note that publishers timing varies and it can take a number of weeks to provide a file, if available.

Important: A receipt is usually requested by publishers before an alternate format file is provided.
